Lab forms will be provided by The Style Dental with just simple call.
Please fill the form with as much detail as possible such as tooth number, shade and type of crown you need.
Packaging
Please send PVS or rubber impressions and Do Not Send Alginate Impressions.
Please take care of impressions to avoid of distortion and check carefully models before sending.
Payment
Invoice will be sent to you on a monthly basis and to be paid within 30 days of receiving your statement. Payment can be paid by direct debit or cheque.
